Documents Required
All foreign-language documents must be translated into Polish by a sworn translator. As of January 2026, applications must be initiated digitally through the MOS Portal.
Identity and General Docs
- Valid Travel Document: Your passport (original for inspection, plus copies of all stamped pages).
- Digital Application Form: Completed and submitted via the electronic system.
- Photographs: 4 recent color photos (35mm x 45mm) meeting biometric standards.
- Proof of Payment: Receipt for the stamp duty (Opłata skarbowa).
Supporting Evidence (Based on Category)
- For Polish Origin: Original vital records (birth or baptismal certificates) of ancestors and documents proving their Polish nationality. Note: The Karta Polaka is not considered proof of origin for this specific sub-pathway; you must provide the underlying historical records.
- For Karta Polaka Holders: A copy of your valid Karta Polaka.
- For Spouses: A marriage certificate issued by the Polish Civil Registry Office (Urząd Stanu Cywilnego) no more than 3 months prior to submission, and a copy of the spouse’s Polish ID.
- For Children: Birth certificates and proof of the parent's legal status in Poland.
