Documents Required
The process involves two distinct stages of documentation: first to obtain the SLA Certificate, and second to obtain the Residence Permit.
For the SLA Certificate
- Proof of Origin: Original birth certificates, baptismal records, or official census/military records showing the nationality of your ancestors up to the third generation.
- Proof of Awareness: A written declaration or testimony from a Slovak cultural organization or two SLA-holding witnesses.
- Criminal Record Check: An original background check from your country of origin and any country where you have lived for more than six months in the last five years. These must be apostilled (or legalized) and accompanied by an official translation into Slovak.
- Identity Photos: Two color photographs (3.5 x 4.5 cm).
For the Temporary Residence Permit
- The SLA Certificate: The original certificate issued by the USZZ.
- Valid Passport: Your travel document must be valid for at least six months beyond your intended stay.
- Updated Criminal Record Check: If your previous check has expired (they are generally valid for 90 days for the police application).
- Proof of Accommodation: A notarized lease agreement or an affidavit (čestné vyhlásenie) from a property owner in Slovakia confirming you have a place to live.
- Identity Photos: Two color photographs (3 x 3.5 cm).
