Do you have a parent, grandparent, or great-grandparent who was a Czechoslovak citizen?
Documents Required
All foreign documents must be apostilled (or legalized, depending on the issuing country) and translated into Slovak by an official translator registered with the Slovak Ministry of Justice.
Identity and Personal Documents
- Valid Passport: A high-quality copy of your current national passport.
- Birth Certificate: Your full "long-form" birth certificate.
- Marriage Certificate: If applicable, to document any name changes.
- Curriculum Vitae: A brief, signed biography written in Slovak.
- Criminal Record Check: A background check (e.g., FBI Identity History Summary in the US) from your country of residence and birth. These must be less than 90 days old at the time of submission.
Proof of Ancestry
- Vital Records: A "paper trail" of birth, marriage, and death certificates linking you directly to the qualifying ancestor.
- Proof of Ancestor’s Citizenship: Evidence that the ancestor held Czechoslovak citizenship. This can include old Czechoslovak passports, military booklets, citizenship certificates (domovský list), or specific census records.
