Documents Required
All documents must be submitted in Polish. Any foreign-language documents must be accompanied by a translation performed by a sworn translator (tłumacz przysięgły) registered in Poland.
Identity and Status Documents
- Official Application Form: A specific template addressed to the President, completed in Polish.
- Photographs: Recent passport-sized photos (typically 1–3).
- Valid Passport: A certified copy of your current foreign passport.
- Residence Documents: If you live in Poland, a copy of your Karta Pobytu (Residence Card).
Vital Records
- Birth Certificate: Original or a certified copy.
- Marriage Certificate: If applicable, to establish current civil status.
- Legalization: Foreign civil status documents must typically be Apostilled or legalized by the issuing country.
Supporting Evidence
- Personal Justification (Uzasadnienie): A detailed statement explaining why you seek Polish citizenship and highlighting your ties to the country.
- Proof of Achievements: Diplomas, professional awards, employment contracts, or evidence of property ownership in Poland.
- Ancestry Proof: Documents proving Polish parentage or grand-parentage if you are applying based on roots.
- Parental Consent: If applying for minor children, the other parent must provide formal consent unless they have been stripped of parental rights.
