Documents Required
All foreign-issued documents must be apostilled or authenticated by the Philippine Foreign Service Post in the country where they were issued. Documents not in English must include an official translation.
Principal Applicant
- Proof of Former Citizenship: Your PSA-issued Birth Certificate or an expired Philippine passport.
- Proof of Naturalization: Your naturalization certificate from your current country of citizenship.
- Identity Documents: Your current valid foreign passport and two original copies of the Consolidated General Application Form (BI Form CGAF-001-Rev 2).
- Letter Request: A formal letter addressed to the Commissioner of Immigration stating your intent to reside permanently.
- Clearances: A Bureau of Immigration (BI) Clearance Certificate and an NBI Clearance (if you have been in the Philippines for six months or longer since your last arrival).
- Medical Clearance: Issued by the Bureau of Quarantine (required for certain nationalities or specific health conditions).
Dependents
- Proof of Filiation: A PSA-issued Marriage Certificate (for a spouse) or Birth Certificates (for children).
- CGAF Form: A separate application form for each dependent.
- NBI Clearance: Required for any dependent aged 15 or older if they have resided in the Philippines for at least six months.
