Was the child born in the Philippines?
Documents Required
Applicants must compile a dossier for the Bureau of Immigration (BI). Any documents issued by foreign governments must be apostilled or authenticated by the Philippine Foreign Service Post in the country of origin.
Identity and Vital Documents
- Consolidated General Application Form (CGAF): BI Form 2014-00-003 Rev 0.
- PSA Birth Certificate: An official birth certificate issued by the Philippine Statistics Authority (PSA). Note that a copy from the Local Civil Registrar (LCR) is generally insufficient if it has not yet been encoded by the PSA.
- Child’s Valid Passport: The original and a photocopy of the child’s current foreign passport.
Proof of Parent’s Status
- Passport Photocopy: Showing the parent’s valid 13-series visa implementation stamp.
- ACR I-Card: A photocopy of the parent’s Alien Certificate of Registration Identity Card (front and back).
- Certificate of Residence: The parent’s latest Immigrant Certificate of Residence (ICR) or Native Born Certificate of Residence (NBCR).
Supporting Clearances and Financials
- BI Clearance Certificate: A valid clearance issued by the Bureau of Immigration.
- Affidavit of Support: A notarized document executed by the parent(s) guaranteeing the financial support of the child while in the Philippines.
