Are you a former citizen of the Czech Republic or the Czechoslovak (Socialist) Republic?
Documents Required
All documents (except your passport) must be submitted in Czech or with an official certified translation. Foreign public documents (like birth or marriage certificates) must be authenticated with an Apostille or super-legalization from the issuing country.
- Identity Documents: A valid original travel document (passport) and two recent passport-sized photographs.
- Proof of Purpose:
- For family: Original or certified copies of birth certificates or marriage certificates.
- For former citizens: Documents proving the date and method of loss of previous citizenship.
- For compatriots: A certificate of Czech origin issued by the Ministry of Foreign Affairs.
- Proof of Accommodation: A notarized lease agreement or a "Proof of Accommodation" form signed by the property owner. If the owner is an individual, their signature must be officially verified (notarized or signed at a CzechPoint).
- Financial Documents: You must show proof of funds demonstrating that your monthly income (combined with any family members living with you) meets the sum of the national living minimum and normative housing costs.
- Criminal Record Extract: An extract from your country of origin and any country where you have resided for more than six months in the last three years. This is not required for applicants under the age of 15.
