Documents Required
All documents issued in a foreign language must be accompanied by a sworn translation into Polish.
- Identity Documents: A valid passport and a photocopy of the identification pages.
- Application Form: A completed application form, written exclusively in Polish.
- Photography: One recent color photograph (35 x 45 mm).
- Proof of Polishness: This is the core of your application. You must provide original documents such as:
- Polish identity documents of your ancestors.
- Birth, marriage, or baptismal certificates that explicitly state "Polish" nationality or citizenship.
- Documents confirming military service in Polish formations.
- Evidence of the "rehabilitation" of ancestors who were persecuted for their Polish identity.
- Supporting Evidence: While the interview is the primary test of language, you may include Polish language certificates or school leaving certificates from Polish-language schools if you have them.
