Documents Required
All foreign documents must be originals or certified copies and must be accompanied by a translation into Polish performed by a sworn translator (tłumacz przysięgły) registered with the Polish Ministry of Justice.
Identity and Vital Records
- Proof of Ancestry: Birth, marriage, and death certificates for every person in the line of descent, starting from the Polish ancestor down to you.
- Proof of Polish Citizenship: Historical documents such as old Polish passports, ID cards (Dowód Osobisty), military booklets, or residency listings (Księgi Ludności).
Supporting Evidence
- Naturalization Records: Official documents from the country of emigration (e.g., US Petition for Naturalization) showing exactly when your ancestor became a foreign citizen. This is vital for determining if the "Military Paradox" applies.
- Change of Name: Official court records or marriage certificates if any ancestors changed their names after emigrating (e.g., Jan becoming John).
