Documents Required
All documents issued outside the Czech Republic must be officially translated into Czech by a certified translator. Depending on the issuing country, documents may also require an Apostille or super-legalization to be recognized as valid.
Identity and Personal Documents
- Application Form: The specific declaration form corresponding to your eligibility section (e.g., Section 31).
- Your Birth Certificate: A long-form version showing parental details.
- Marriage/Divorce Certificates: Required for every generation in your lineage to prove legal name changes and the link between generations.
- Criminal Record Check: Required from your country of residence for all applicants aged 15 and older.
Ancestral Proof
- Proof of Ancestor’s Citizenship: This is the core of the application. You must provide documents showing your ancestor held Czech/Czechoslovak status, such as:
- Old Czechoslovak passports or national ID cards (občanský průkaz).
- Certificates of Domicile (domovský list).
- Military records or birth certificates from the Czech archives.
- Proof of Citizenship Loss: Documents showing when and how your ancestor lost their Czech citizenship, such as a foreign Certificate of Naturalization. This confirms they did not lose it in a way that disqualifies their descendants.
